For the past two years, H.A.T.S. has proudly connected individuals and families throughout our community with essential healthcare, housing, enhanced care management, transportation, and compassionate outreach services!
1 Year Anniversary 10/01/2025
Our ECM/CS Lead Care Managers and Housing Navigators have served over 800 Medi-Cal members and we have helped over 200 people attain housing!
