Enhanced Care Management (ECM) is a specialized, whole-person care program tailored for individuals facing higher, more complex health and social challenges. Our team works directly with you in your community to coordinate every aspect of your care. We partner directly with your Managed Care Plan to streamline your entire health journey—from organizing medical visits to connecting you with critical community resources.

Our Community Supports (CS) program delivers hands-on assistance to Medi-Cal members who need dedicated help finding, securing, and keeping a place to call home. From searching for accessible rentals to helping relieve initial financial burdens, our advocates walk alongside you every step of the way to ensure long-term housing stability and independence.
